A federal investigative board has released a final report finding that a 2013 train derailment and chemical explosion in Maryland was caused by a truck driver who failed to yield at the rail crossing.
The National Transportation Safety Board on Wednesday determined that the dump truck driver, John Alban Jr., was at fault in the 15-car derailment and subsequent explosion in Rosedale. Alban has said he was on the phone using a hands-free device at the time of the crash.

The NTSB issued recommendations that include prohibiting commercial vehicle drivers from using hands-free devices.
The board also said overgrown vegetation and inadequate federal oversight of Alban's trash hauling company, Alban Waste LCC, contributed to the collision. The company had an above-average number of violations, resulting in trucks being ordered off the road.
